Mission:

Our mission is to provide an educational environment whereby the Certified Surgical Physician Assistant is trained in the highest level of patient care. Training will be provided in the Cardiac intensive care unit, surgical intensive care units, clinics, on the surgical floors and in the operating room based on the tract chosen (Cardiac Intensive Care, Surgical/Academic or Surgical/Community hospital based) and will involve pre-operative, intra-operative, intensive care and post-operative care. In an academic environment, the resident will be exposed to basic and advanced intensive care and surgical training concepts and techniques. Under appropriate supervision the resident will gain the confidence and experience necessary to practice in a wide variety of intensive care and surgical specialties in academic and private settings.

Program Description

There are 3 twelve-month tracks to choose from:

  • The Johns Hopkins Hospital Postgraduate Surgical Residency for Physician Assistants/Cardiac ICU. Time will be spent primarily in the Cardiac Surgical Intensive Care Unit. Residents will have one elective surgical rotation and opportunities for Cardiac OR experience.
  • The Johns Hopkins Hospital Postgraduate Surgical Residency for Physician Assistants/Surgical ICU. Time will be spent primarily in the Surgical Intensive Care Units. Residents will have one elective surgical rotation on a general surgery service.
  • The Johns Hopkins Hospital Postgraduate Surgical Residency for Physician Assistants/General Surgery. Time will be spent in several surgical subspeciality rotations with at least one elective in an Intensive Care Unit. One rotation will be in a community based hospital.
  • General Surgery rotations will be based on the track that you are selected for and may include:

    Responsibilities:

    Expectations of PA resident include, but are not limited to:
    • Performing H&Ps
    • Writing admission orders
    • Dictating discharge summaries
    • Performing blood cultures and ABGs
    • Ordering diagnostic tests and performing proper interpretation
    • Completing patient assessments
    • Assisting the residents and attendings in all aspects of patient care

    In the Cardiac Surgical Intensive Care Unit, you will become a full and active member of a multi-professional team led by an Anesthesia Critical Care Intensivist. While in the ICU you will learn to interpret critical caradiac data, utilize monitoring and support devices (including balloon pumps, pacemakers, and ventricular assist devices), and initiate appropriate therapies for patients recovering from or requiring cardiac surgery and/or cardiac/lung transplantion. Advanced procedures such as central line, arterial line, and chest tube placement are performed under supervision until competency is achieved. Residents will be responsible for the compresensive evaluation and management of their patient group.

    In Surgical Intensive Care Units you will become a full and active member of a multi-professional team led by both Surgical and Anesthesia Critical Care Intensivists. While in the ICU you will learn to interpret critical hemodynamic and pulmonary data, utilize monitoring and support devices (including Pulmonary Artery Catheters and Oscillating Ventilators), and initiate appropriate therapies for patients recovering from or requiring general surgical interventions. ICU support is provided to patients on the Trauma, Transplant, Thoracic, Endocrine, GI, Hetobiliary, Plastics, Craniofacial, Orthopedic, Head and Neck, ENT, as well as general surgical services. Advanced procedures such as central line, arterial line, intubation, and chest tube placement are performed under supervision until competency is achieved. Residents will be responsible for the comprehensive evaluation and management of their patient group.

    When on a surgical rotation you will prepare patients for surgery, prep, drape, first assist on appropriate cases or second assist in the OR (depending on track selected) and follow patients post-operatively. You will perform phlebotomies, place IV lines, Foleys and remove surgical drains. Advanced procedures such as central lines and chest tube placements and ICU specific skills will be performed under supervision until hospital competency is obtained.

    PA Residents will follow the same schedule and adhere to the same call requirements as the surgical residents on the team. Both physician assistant and surgical residents will follow the guidelines established by the Accreditation Council for Graduate Medical Education that became effective on July 1, 2003.

    Physician Assistant Residents will be taught core clinical skill competencies in the Minimally Invasive Surgical Training Center (MISTC) a 6200 square foot laboratory facility under the medical direction of Dr. G. Melville Williams.

    Stipend

    The Johns Hopkins Hospital Department of Surgery Postgraduate Surgical Residency for Physician Assistants will be a twelve month employment contract. The Stipend is $40,000.00 with twenty-two days of personal time off (PTO) to be used as vacation or sick time. Parking will be subsidized at fifty percent and meal vouchers during call will be provided. Health and dental insurance, life insurance, short and long-term disability is available. Physician Assistant Surgical Residents will be subject to the guidelines established by the Accreditation Council for Graduate Medical Education. DEA, Maryland Controlled Dangerous Substance number, License fee, and delegation fee will all be paid for by the hospital. A minimum of fifty category one and two credits will be available at no cost through various academic requirements during the residency.

    Faculty

    The faculty for the Postgraduate Physician Surgical Residency for Physician Assistants includes the core faculty of the Intern and Resident Lectures.

    Dr. Freischlag came to Johns Hopkins from the University of California in Los Angeles, where she was Chief of the Vascular Surgery Division and Director of the Gonda (Goldschmied) Vascular Center. Dr. Freischlag completed her surgical residency and post residency Vascular Fellowship at UCLA. Before Dr. Freischlag returned to work at UCLA, she was Professor of Surgery at the Medical College of Wisconsin where she served as the Vice Chair of the Vascular Surgery Section and Chief of Surgery at Zablocki VA Medical Center.     Dr. G. Melville Williams, M.D. is the medical director for the Physician Assistant Residency Program. He began his academic career as an undergraduate at Oberlin College and then went on to attend Harvard Medical School in Boston in 1957. He did his surgical residency at the Massachusetts General Hospital, finishing in 1964. Dr. Williams spent a year learning immunology as a special fellow of the NIH in Australia. Upon his return to the states, he joined the group pioneering renal transplantation at the Medical College of Virginia and took a position as Director of Surgical Research. In 1969 he came to Johns Hopkins as the Chief of Transplant Surgery, a position that he held until 1995. Dr. Williams also served as Chief of Vascular Surgery in 1973 through 2002. Dr. Williams now focuses on research related to the ischemia-reprofusion injury. He truly enjoys mentoring surgical residents, young faculty members, and espeically the Physician Assistant Residents.

    Program Requirements

    • Graduation from an ARC-PA (Accreditation Review Commission on Education for the Physician Assistant, Inc.) approved PA Program, or a current second year student in a Physician Assistant Masters program.
    • Masters preferred or evidence of commensurate work experience.
    • GPA > 3.4 on a 4.0 scale.
    • Eligible to sit for PANCE by September. Preference will be given to applicants who have taken and passed the PANCE exam.
    • Applicants will also be evaluated on prior work experience, volunteer experiences, references, and interviews.
    • Interview (The top 25 candidates will be invited for interviews at The Johns Hopkins Hospital.)

    Recent graduates can now take the PANCE as early as seven days after their official graduation date. Tests are now given throughout the year and results are usually available two weeks after the test. Students must be able to pass the PANCE within 30 days after graduation.     Train at the Johns Hopkins Medical Institutions for a career in Emergency Medicine. The residency will be based at the Johns Hopkins Bayview Medical Center.

    This will be an 18-month residency with the following proposed curriculum:

    • Nine months in the EM at JHBMC
    • One month rotations: MICU, CICU, Internal Medicine, Peds ER
    • Two week rotations: Ophthalmology, Orthopedics, Neurology, Burn Unit/Line Service, Ob/Gyn, Anesthesiology, Radiology, Electives
    • Attend weekly Emergency Medicine Residency conferences at the Johns Hopkins Hospital. While on off-service rotations, will attend specialty specific conferences.

      Receive training in ATLS (pending) and Emergency Ultrasound.

      There will be a stipend paid. Life, AD&D, Short-term Disability, Malpractice provided. Medical, Dental, Vision plans offered. CME funds. Paid time off.

      We will be accepting one resident every six months. During final six months, resident will also function as mentor to junior residents. We are planning to have the first resident begin in March 2007.
